Below are the details straight from the Tyson site:

1. Purchase Tyson® Chicken Nuggets and/or Tyson Crispy Chicken Strips, during the promotional period, May 1, 2011 through May 31, 2011.
    a.Purchase 1 (one) qualifying product and receive a Free KODAK Photo Book.
    b.Purchase 2 (two) qualifying products and receive a Free Fun Pass.
    c.Purchase 3 (three) qualifying products and receive a Free Salon Treatment.
2. Visit the Redeem tab in order to print your Redemption Form. You will need your UPC code(s) to access the form.
3. Mail the completed form along with your original receipt(s) (copies not accepted) with qualified participating product(s) highlighted to:

Upon receipt, please allow one (1) week for verification. You should receive the gift you have qualified for within 4-6 weeks of verification.

Please note your qualified Tyson® product(s) purchase must be made between May 1, 2011 and May 31, 2011. Your response must be postmarked no later than June 15, 2011 in order to receive your gift. All forms received that are postmarked after June 15, 2011 are not eligible.
